Evergreen Medical Wellness utilizes a membership structure for most services. This helps keep costs for visits, testing, and medications as low as possible while also keeping costs consistent- so you know what to expect. It also gives peace of mind; if you have a concern or question you can reach out by text/phone/portal message at any time for a quick response from your clinician. 

Memberships can be cancelled at any time, though we request that your cancellation request be done in writing, which can be done by text, email, or patient portal messaging (for which you will receive a confirmation of receipt), and that you give 30 days notice which will allow us time to transition your treatment as appropriate.
